If the company sponsors an employee to do professional examination or training, the company may wish to recoup costs associated with if the employee leaves before completing the course or within a short period of time of completing the course (e.g. accountancy exams, CIPD exams etc). This form documents the costs and details the level of repayment to be made. It also authorises deduction from pay for some payments.
